Disability Access Services is moving to a new accommodation software that will better serve the needs of our students and community. This is exciting news; however, it means that the current system is going offline while the new system comes online. DAS will be unable to accept accommodation requests until July 1. Documentation Considerations

DAS considers three resources in determining reasonable accommodations:

  • Self-report
  • Previous academic accommodations (such as an IEP, a 504 plan, approved accommodations at a former college)
  • Formal documentation from a qualified professional

The purpose of the documentation is to establish that the student is a person with a disability, and it should identify and speak to the need for reasonable accommodations.

Documentation should include:

  • A diagnosis of the disability/health condition, or, if a diagnosis is not yet available, a description of symptoms
  • A description of the current impact/limitations of the disability/health condition with specific focus on barriers to the educational and/or housing environment
  • A note on whether the disability/health condition is permanent or temporary
  • Identifying information about the healthcare provider:
    • Contact information
    • License number
    • Signature (electronic or physical)

Types of Documentation

  • DAS Healthcare Provider Form 
  • Official visit summary, such as office visit notes or emergency discharge papers
  • Neuropsychological assessment
  • Individual Education Plan (IEP)
  • 504 Plan
  • Psychoeducational evaluation (for Learning Disability/ADHD/Traumatic Brain Injury)
  • Audiogram
  • Vision screen results
  • Letter from qualified healthcare on provider’s professional letterhead

Questions about Documentation

  • WHO – The Director of Disability Access Services is the only one who reviews documentation. You do not need to send disability documentation to your professors or admissions counselor.
  • WHAT – Students who do not have access to documentation are still encouraged to schedule a meeting with DAS.
  • WHEN – Most accommodation requests cannot be approved until after documentation has been submitted.
  • WHERE – You can submit your documentation directly to your Accommodation Request form, or email it to DAS@cui.edu, or deliver it to the front desk of the Student Success Center (in the Schroeder Library & Learning Commons).
  • WHY – Documentation is essential to establishing that the student can be considered a person with a disability and may need reasonable accommodations to have equal access to their education.
  • HOW – If you need local resources that assess for disabilities and provide documentation, please email DAS@cui.edu.
